INITIAL CONSULTATION

Service Description

An initial assessment will be done during your first visit. The initial visit usually requires an hour of your time. On your first visit remember to bring the following: your physician's name and phone number, list of your current medications, and if applicable the name and contact numbers of other healthcare professionals that you are currently consulting.

Cancelation Policy

Please let us know atleast 24 hours before your appointment if you want to re-sched or cancel it. There will be a $25 fee for late cancellation.
